About Your Therapist
Takiya Bradley, MSW, LCSW

Takiya earned her Bachelor’s degree in Psychology and Master of Social Work from Metropolitan State College of Denver. She has experience supporting clients with anxiety, depression, trauma and PTSD, stress, relationship challenges, and life transitions.
​
Treatment approaches may include Cognitive Behavioral Therapy (CBT), Solution-Focused Therapy, and other evidence-based methods tailored to your unique goals.
Regulation
Mental health professionals in Colorado are regulated by the Colorado Department of Regulatory Agencies (DORA), Division of Professions and Occupations. Clients may contact DORA with questions or concerns regarding professional conduct.
Your Rights as a Client​
You have the right to:

• Ask questions about therapy methods or techniques
• Understand fees and billing policies
• Request or decline specific treatment approaches
• Seek a second opinion
• Stop therapy at any time
• Receive respectful, professional care
• Have your privacy protected

Sexual or romantic relationships between therapist and client are never appropriate or permitted.
Fees & Payment​
Session Rates
​
Initial Intake (60 minutes): $150
Individual Therapy (53 minutes): $150
Couples Therapy (53 minutes): $175
Extended Couples (up to 90 minutes): $200
Paperwork (FMLA/Disability): $100
Late cancellation or no-show: $75
In-person administrative fee: $9.75

Payment is due at the time of service. A card may be kept on file for convenience.
​
If insurance does not cover services, you remain responsible for payment. Boundary and Grace LLC is not a Medicaid provider.
​
Legal services such as court testimony or reports are billed at $425 per hour.
Cancellation Policy​
Appointments cancelled with less than 48 hours’ notice or missed without notice are subject to a $75 fee. Insurance does not cover cancellation fees.
If you arrive more than 15 minutes late, your session may be cancelled and the late fee may apply.

Repeated missed appointments may result in termination of services.
Confidentiality
Your privacy is extremely important. Information shared in therapy is confidential and protected by law.
​
There are limited legal exceptions. Information may be disclosed if:
• You are at risk of harming yourself or someone else
• Child abuse or neglect is suspected
• Abuse or exploitation of a vulnerable adult is suspected
• A court orders disclosure
• Professional consultation or supervision is necessary

Electronic communication (text, email, phone) cannot be guaranteed to be fully secure, though reasonable safeguards are used.
Communication & Availability​
Office hours are Monday through Thursday, 4:00pm–9:00pm.
​
Calls, texts, emails, and client portal messages are typically returned within one business day.
​
Boundary and Grace LLC does not provide emergency or after-hours crisis services.
​
If you are experiencing an emergency, call 911, visit the nearest emergency room, or call 988 for the Colorado Mental Health Crisis Line.
Telehealth​
Telehealth sessions may be available using secure, HIPAA-compliant platforms. A separate consent form is required.

Recording sessions (audio or video) is not permitted.
Social Media​
To protect your privacy and maintain professional boundaries, therapists do not accept friend or follow requests on personal social media accounts. Therapy-related concerns should not be discussed through social media platforms.
Records & Privacy Practices​
Client records are stored securely and retained for seven years after your last session. You may request copies of your records at any time.

Records may be stored electronically using encrypted, HIPAA-compliant systems.
Couples & Family Therapy
When working with couples or families, the relationship is considered the client. Information shared individually that impacts treatment may need to be discussed with all involved parties to support the therapeutic process.
If you prefer complete confidentiality, individual therapy may be recommended.
Minors
Parents or legal guardians typically must consent to services for minors. Court orders and custody agreements may affect treatment decisions. Therapists cannot provide custody recommendations.
Professional Boundaries​
Therapy is a professional relationship. To protect your care:

• Therapists cannot engage in friendships or business relationships with clients
• Bartering or trading services is not permitted
• Romantic or sexual relationships are strictly prohibited
Termination of Services
Therapy may be discontinued at any time by you or your therapist. If therapy is not effective, attendance is inconsistent, or payment issues arise, referrals may be provided for continued care elsewhere.
Consent​
By beginning services, you acknowledge that you understand these policies, your rights, and your financial responsibility, and you consent to treatment with Boundary and Grace LLC.
